Students who must take medication at school must send the medication to school in its original container and must have the appropriate form brought to school with it. These forms can be printed from this site. The Prescription Medication Form must be signed by the physician prescribing the medication. The OTC Medication Form is used when over the counter medicine is sent to school. The form and medication must be brought to the clinic where it will be kept in a locked cabinet until it is administered. A log is kept of all medications given at school. Over the counter medication will not be given more than 10 times without a physician's signature.
