When you are ready to enroll, click on the How to Enroll tab and click on the red Enroll Now button to login.

Your login is your Social Security Number – no dashes. Your pin is the last four digits of your social security number and your full date of birth (MMDDYYYY).

Ameritas is the new carrier for Dental and Vision insurance plans. Ameritas sent out new id cards the first week of September to all dental and vision plan members. Contact Ameritas if you need your member id card at 800-487-5553
