Required Documents for New and Returning Students

Required Documents for NEW and returning Kindergarten -12th Grade Students

  • Certified Copy of Student’s Birth Certificate
    • If a certified copy of the student’s birth certificate cannot be provided, you may submit one of the following documents until a copy of the certified birth certificate is obtained:
      • Current Passport or other government-issued documentation
      • Adoption Documentation
      • Hospital Birth Records, or
      • Additional official legal documentation establishing the student’s identity
  • Student Social Security Number, providing a copy of the card is optional
  • Government-issued Parent or Guardian Photo Identification (e.g., Driver’s License, Passport, or Texas State ID, a matching residence address is preferred)
  • Current Immunization Record or Valid Medical or Conscientious Exemption Form.

  • Previous School/District Withdrawal Documentation
  • Special Services/Program and Plan Documentation - (Including Intervention Plans, Individualized Education Program (IEPS), LPAC documentation, and Section 504 Plans), if applicable.
  • Previous school records and current/most recent report cards
  • Unofficial transcript, if applicable
  • Legal Documentation - (Signed Court Orders, or other legal documents submitted in full), if applicable.
  • Department of Family and Protective Services Documentation, if applicable.
  • Required Proof of Residency
    • Families Building or Purchasing a Residence Within EMS ISD.
      • Building a home: Provide a builder's contract that includes the residence address and the projected completion date; or
      • Purchasing a home: Provide a purchase contract that includes the residence address and the projected move-in date.
      • Utility Bill: Provide proof of utility service dated within the last 30 days that includes the name of the owner, service dates, and the residence address. Acceptable documents include electric, gas, or water bills. 
    • Families Who Own or Rent a Residence Within EMS ISD
      • Provide a signed rental or lease agreement that includes the residence address, names of all residents (including children), the lease start and end dates, and the signatures of both the lessor and lessee. 
      • Provide proof of utility service dated within the last 30 days that includes the name of the renter/tenant or owner, service dates, and the residence address. Acceptable documents include electric, gas, or water bills. 
    • Unacceptable Documents include cellular phone bills, insurance bills, cable bills, and utility service request or termination notices. 
    • Families Residing with Another Person Within EMS ISD
      • ​​​​​​​Families living with another individual who resides within the district must complete an Affidavit of Legal Residency. This legal enrollment form must be completed and notarized annually.
