Eagle Mountain Saginaw Rank One Online Athletic Paperwork Instructions

These forms must be completed prior to athletic participation for the upcoming school year.

To complete the required medical forms, please follow these instructions:

1. Go to https://eaglemountainisd.rankonesport.com/New/Home.aspx
2. Click on the tab that says “Electronic Participation Forms”
3. Scroll to the bottom of the page, where you should see this:
4. Once you click on this button, you will be redirected to a page, which asks, “Have you previously created a Parent Account when completing forms online?” followed by several options. Search the option that fits you best and go from there!
5. Please make sure that you complete the “Medical History”, “Sports Participation Packet”, and the “Emergency Card”. You will be able to see Approved/Incomplete out to the side, so there should be no need to call and check on form status

Instrucciones

1. Vaya a rankonesport.com
2. Haga clic en la sección, “Parents/Students Fill out Electronic Forms” (“Padres/Estudiantes Completen formularios electrónicos” en el medio en amarillo)
3. Seleccione 'TX' y luego Eagle Mountain-Saginaw
4. Siga las instrucciones con cuidado para completer DOS formularios : (Mire debajo de “Electronic Participation Forms”) y complete:

a. Formulario de historial medico (“Medical History”)

b. paquete de participación deportiva (“Sports Participation Packet”)

c. tarjeta de emergencia (“Emergency Card”)

Se requiere el número de identificación del estudiante para completar. Metan su correo electrónico para recibir una confirmación, diciendo que los formularios han sido enviados. 

5. Imprima del sitio web y complete:
   1. El formulario de historia médica (necesita firma de los padres y el estudiante)
   2. El formulario de examen físico (necesita ser llenado por el médico).
6. Entregue estos DOS documentos directamente al entrenador medico de deportes de la escuela secundaria o coordinador de la escuela intermedia.
7. Explique todas las respuestas de "sí" en el espacio provisto en el formulario de la historia médica.

TODOS LOS ESTUDIANTES DE GRADO 7-12 serán REQUERIDOS COMPLETAR UNA EVALUACIÓN FÍSICA / MÉDICA ANUAL con una FECHA DEL 1^o DE MAYO o después del 1º de mayo.
